Postoperatory Euglycemic Diabetic Ketoacidosis: A Not So Rare Complication of Sglt2 Inhibitors

چکیده
Diabetic ketoacidosis is one of the most well-known complications diabetes mellitus and potentially fatal. It characterized by a state hyperglycemia (glycemic levels >300mg/dL), associated with metabolic ketoacidosis. has been described that up to 3% diabetic can occur blood glucose below 250 mg/dL, an incidence increased since introduction SGLT2-i. We report case female patient in her seventies, history hypertension, dyslipidemia, ischemic heart disease type 2 treated insulin, metformin canagliflozin. During post-operative period elective surgery, she presented decreased level consciousness acidosis. She was admitted ICU required invasive mechanical ventilation vasopressors. Euglycemic diagnosed after treatment, discharged no sequelae.

Euglycemic Diabetic Ketoacidosis: A Predictable, Detectable, and Preventable Safety Concern With SGLT2 Inhibitors.
Recently, the U.S. Food and Drug Administration (FDA) issued a Drug Safety Communication that warns of an increased risk of diabetic ketoacidosis (DKA) with uncharacteristically mild to moderate glucose elevations (euglycemic DKA [euDKA]) associated with the use of all the approved sodium–glucose cotransporter 2 (SGLT2) inhibitors (1). This Communication was based on 20 clinical cases requiring...

It is possible that SGLT2 inhibitors trigger euglycemic diabetic ketoacidosis in some patients. Possible mechanism of euglycemic DKA induced by SGLT2 inhibitors is illustrated.

Diabetes mellitus is a worldwide disease that leads to several acute complications including diabetic ketoacidosis, hyperosmolar hyperglycemia, and hypoglycemia. In addition, diabetes causes many chronic complications that lead to debilitation and diminished quality of life. Diabetic ketoacidosis is one of the serious acute complications; however, it is usually preceded by infection, acute myoc...
متن کاملEuglycemic Diabetic Ketoacidosis: A Potential Complication of Treatment With Sodium–Glucose Cotransporter 2 Inhibition
OBJECTIVE Sodium-glucose cotransporter 2 (SGLT-2) inhibitors are the most recently approved antihyperglycemic medications. We sought to describe their association with euglycemic diabetic ketoacidosis (euDKA) in hopes that it will enhance recognition of this potentially life-threatening complication. RESEARCH DESIGN AND METHODS Cases identified incidentally are described. RESULTS We identif...
متن کاملSGLT2 Inhibitors: A Systematic Review of Diabetic Ketoacidosis and Related Risk Factors1
Background: Sodium glucose cotransporter-2 (SGLT2) inhibitors are recommended by the American Diabetes Association (ADA) for the treatment of type 2 diabetes as one of six second-line therapy options after initial therapy with metformin. In addition, they have been studied in combination with insulin as a potential option for treating patients with type 1 diabetes. These medications act by inhi...
